All health care facilities and providers must report cases of malignant disease and benign brain tumors diagnosed, treated, evaluated or medically supported, that have not been reported previously to the Cancer Registry. All health care facilities and providers who provide diagnosis, treatment, evaluation or medical support must report further demographic information or treatment information about the patient to the Cancer Registry.
