“Admissions, Transfers and Discharges”
 
The law establishes the procedures for admissions, transfers and discharges of patients.  Facilities shall admit and care for only those individuals in need of long term care services for which they can provide care and services appropriate to the individual's physical, emotional and social needs. Prior to admission, an individual's needs shall be evaluated and alternative care plans considered.  Patients shall be admitted only on the written order of a physician, physician assistant, or nurse practitioner.  Facilities must exchange information on resources and services with other agencies and institutions that provide health care in their area. The law also establishes the procedure for admission of mental health patients. Summaries or reports of psychiatric evaluations and examinations, including current status, recommendations for management and medications, are sent to the facility and maintained with the patient's or resident's clinical record.In addition, any transfers or discharges are entered into a written form agreement that ensures timely admission and provisions for continuity.
