A sponsor for certified adult family homes is required to maintain a record for each resident, and these records are to be kept in a secure place inside the home. The resident or resident’s guardian should be allowed to access his/her record upon request, but otherwise the record must be kept confidential. Further, the law specifies what each record should contain: name and date of birth; name and contact information of guardian, if applicable; name and contact information of everyone who needs to be notified in case of emergency; name and contact information of referring agency and service coordinator; insurance identification numbers and name of pharmacy resident uses; agreement for services; medical examination report; individualized service plan; evidence that resident and resident's guardian received notice of resident's rights. The sponsor is required to comply with all DHS requests for information about residents, services and the operation of the home.
