Wis. Admin. Code DHS §34-24 - Client Service Records - Emergency Mental Health Service Programs

This law requires that emergency mental health service programs maintain accurate records of all the services provided to clients, including notes and crisis plans.  The law makes the program administrator responsible for the maintenance and security of these records. 
 
The law states that these records must be kept in a central place not accessible to persons receiving care through the program, and that they be kept safe and secure, in accordance with standard practices for maintenance of records.  The law requires that organizations that provide these services under contract with the county establish written policies for the maintenance and disposal of these records in case the program terminates.  The law further requires that these records be maintained confidentially.
