Patient's revocation of authorization for disclosure
A patient that authorizes their health care provider to disclose their medical information may revoke this authorization at any time through writing. However, revocation is not effective if disclosure is necessary for payment purposes or a substantial action has been taken relying on the authorization. Providers that disclose information pursuant to an authorization in good faith and without notice of revocation are immune from any resulting liability.
