Certification of the Patient Health Record
Upon a request by the person requesting the record, the provider or facility must certify the record furnished and may charge for such certification. The certification should be attached to the record, and should disclose the identity of the patient, the kind of health care information involved, the identity of the person to whom the information is being furnished, the identity of the health care provider or facility furnishing the information, the number of pages of the health care information, and the date on which the health care information is furnished.
