Disclosure of Health Care Information by a Health Care Provider

Health care providers, their agents, and any individuals who assist health care providers may not disclose health information about a patient to any other person without the patient’s written authorization.  A patient has the right to receive an accounting of the disclosures made by a health care provider to other parties for the six years preceding the request, except for disclosures to carry out treatment or payment, to persons involved in the patient’s care, for national security or intelligence purposes, to correctional institutions, or to parties that are authorized by the patient to receive health care information.
