Hospital Management of Patient Information
Hospitals must provide medical staff, employees and other authorized persons with access to patient information systems, resources, and services. Staff must maintain the confidentiality, security and integrity of the information in patient records. Every patient assessed or treated must have his or her own medical record that identifies the patient, contains clinical data to support the diagnosis, course and results of treatment, contains signed consent documents, promotes continuity of care, contains accurately written, signed, dated and time entries, and indicates authentication after the record is transcribed. Hospitals must have in place a process to review records for completeness, accuracy and timeliness.
