Transfers and Discharges - 10-144-110 Me. Code R. § 19.F

When a resident is transferring within a Skilled Nursing Facilities, the current resident record may be continued.  However, before a facility transfers or discharges a resident from one facility to another facility, institution or agency, the facility must prepare a referral form.  The form should be forwarded at the time a resident is transferred, but a copy must be kept for the resident’s record.  To ensure optimal care, the referral form should contain an appropriate summary of information about the discharged resident.
