The governing body of each facility providing mental health, mental retardation, and/or alcohol and drug treatment services must ensure that an individual client record is maintained for each client being served, which must include the following information:
- Identifying information about the client, including the client’s sex and age;
- The date of the client’s admission to the facility;
- The source of the client’s referral to the facility;
- A written fee agreement between the client and the facility, if the facility charges for services; and
- Appropriate consent and authorization forms for the release or obtainment of information about the client.
Records requirements for different types of mental health and mental retardation facilities and programs are contained in the Tennessee Complete Rules and Regulations, Chapter 940-5.
