Each patient of a licensed dentist will be afforded the following rights:
- To have their records kept confidential and private;
- To be provided information concerning their diagnosis, evaluation, treatment options and progress;
- To an opportunity to participate in decisions involving their health care;
- To refuse any diagnostic procedure or treatment and be advised of the consequences of that refusal; and
- To obtain a copy or summary of their personal dental record.
