Each induced termination of pregnancy must be reported to the Office of Vital Records within 10 days after the procedure by the person in charge of the institution where the procedure took place. If the abortion was performed outside of an institution, the attending physician must make the report. The individual undergoing the procedure must not be identified by name on the report, but some unique identifier must be used so that further information can be retrieved if necessary.
