“Requirements under the insurance law”
Health insurers must provide enrollees with the following information:
- An explanation of benefits;
- Any benefit limitations, reductions, exclusions or exceptions to covered services;
- The nature of any payments required an enrollee such as copayments or deductibles;
- The nature of any limitations of payment by the insurer, including limitations based on the use of network or out-of-network health care providers; and
- A description of its grievance procedures.
