An adult patient who inspects his or her records and finds that there is an item that is incomplete or inaccurate has the right to provide a written addendum to the health care provider. The addendum is limited to 250 words per incorrect or inaccurate item and must clearly state that the patient wants the addendum to be made part of his or her record. The provider must attach the addendum to the patient’s record and include it whenever a patient’s medical record is disclosed to a third party.
