A patient may revoke their do not resuscitate order (DNR) at any time by expressing their intent to any physician or nurse in the hospital. A surrogate, parent, or legal guardian may revoke a DNR by providing a physician or nurse at the patient’s hospital with a signed and dated revocation document or by orally revoking the DNR in the presence of the attending physician and an adult witness.
Physicians must immediately cancel a DNR after receiving a valid revocation, document the revocation in the patient’s medical chart, and alert all persons caring for the patient of the revocation.
