Records and documents that relate to allegations and investigations of patient abuse or mistreatment at a facility that provides services to the mentally disabled may be released to qualified persons upon request. The names and identifying information about patients and employees in such records and documents will not be included without the consent of the patient(s) and/or employee(s). Such records may not be further released by the qualified recipient unless otherwise authorized.
