The commissioner of health has discretion to establish health homes to provide services to Medicaid beneficiaries that have chronic conditions. If the commissioner exercises such discretion, they must create standards for home health services, define payment methodologies, establish eligibility criteria for Medicaid beneficiaries, and assign eligible Medicaid beneficiaries to a health home provider.
