The state maintains an immunization data registry to collect, store, analyze, release and report immunization data. Providers may provide immunization data to the registry unless the patient or the patient’s parent or guardian completes an immunization data exemption form. Records maintained as part of the registry are confidential. The state may only release the information to the individual or his or her parent or guardian, the immunization data registry of another state, a provider, a local health department, an elementary or secondary school attended by the individual, a licensed child care center attended by the individual, a child placing agency, or a college or university attended by the individual. However, the state may release summary statistics regarding information in the registry if the summary does not reveal the identity of the individuals.
