The department of mental health must keep in its office, accessible only to its employees, a record with the following information for every patient in the institutions governed by the department:
- Name;
- Residence;
- Sex;
- Age;
- Nativity;
- Occupation;
- Condition;
- Date of entrance or commitment;
- Date, cause and terms of discharge;
- Condition of such person at the time of leaving the institution;
- All transfers from one institution to another;
- Date and cause of death, if such person dies while in the care or custody of the department.
In the case of an accident or injury or peculiar death of a patient, the managing officer of an institution will make a special report to the department within 24hours, giving the circumstances as fully as possible.
