As part of the quality assessment and performance improvement program required of all health care facilities (HCF), each ambulatory surgical facility must collect and report the following data to the director of health:
- The total number of patient visits;
- The total number of patient transfers to a hospital and the reason why;
- The total number of deaths in the facility; and
- The total number of deaths resulting either from the surgery or from surgical complications that occur in the facility.
