The department of elderly affairs must notify a person of their eligibility to participate in the long-term home health care program. An elderly person that requires nursing home care or their representative may notify the department of affairs of their desire to reside at home or that a physician has deemed them capable of remaining at home or in the care or a responsible adult. The department may subsequently order an assessment to determine whether the elderly person can receive adequate care at home. If the assessment indicates such care is possible, then a case manager must develop plan of care for the elderly person.
