The Department of Social Services must establish a pilot program in which a primary care provider will coordinate the health care services provided to HUSKY Plan, Part A beneficiaries. A maximum of 1000 beneficiaries may participate in the pilot program. Each beneficiary will either select or be assigned the primary care provider responsible for coordinating their care.
