A hospice must provide each patient or patient representative with a written statement of rights before services are provided. The statement must inform patients of their rights and hospice rules, right to prepare an advance directive, right to be informed of significant changes, and the hospice’s discharge criteria. Additionally, patients have the following rights:
- To receive pain management and symptom control;
- Participate in care planning;
- To select or refuse treatment;
- To choose his/her physician;
- To confidentiality of personal and clinical information;
- To receive a copy of own clinical record;
- To be free from mistreatment, neglect, or abuse;
- To be free from restraints or seclusion, unless authorized by a physician;
- To be treated with courtesy and respect;
- To privately communicate with others;
- To receive and refuse visitors;
- To be informed of the types of services available, prior to admission;
- To be informed of services that are charged for, and the charge itself.
All patients have the right to make a complaint to the hospice or Department of Health Services, and be advised of the consumer service contact information.
