Government funded long term home health care services will be provided in accordance with a comprehensive evaluation of a applicant’s “medical, social, and environmental needs.” The comprehensive evaluation will be used to form a plan of care for applicants. The director of the department of human services will designate persons to complete the assessment for hospitalized applicants.
