A physician who has diagnosed a child under 18 years old with Type I or Type II diabetes must report to the Oregon Health Authority the following information:
- The name and address of the child;
- Age of the child;
- Date of birth of the child;
- Type of diabetes; and
- Date of diagnosis or first treatment.
