When the Department of Human Services or a type B area agency on aging discloses a file, form or report to the public related to adult foster home licensing, complaints, or investigations, the department or agency may not disclose the following information about residents:
- The name of a resident;
- Demographic or other information that can be used to identify a resident;
- Health information that relates to past, present or future physical or mental health or condition of a resident;
- Health information that relates to the provision of health care to a resident;
- Health information that relates to past, present or future payment for the provision of health care to a resident.
