Licensed general acute care hospitals and ambulatory surgical facilities must file with the Office for Oregon Health Policy and Research, or its designee, ambulatory surgery data records for each surgical outpatient discharged from its facility. Failure to comply with these reporting requirements may result in civil penalties.
