Every home health agency must maintain a clinical record for each patient that must include the patient’s identifying information, clinical notes, medical history, discharge summary, and the patient’s plan of treatment. Clinical records are the property of the home health agency, and each agency must have written procedures for access, maintenance, retention, and storage of clinical records. Clinical records must be maintained by the agency for 10 years after the date of the patient’s last discharge from the agency and must be stored in a safe and secure environment. Information regarding the patient’s condition and treatment can be disclosed from a patient’s clinical record to the patient upon request.
