Practitioners must report diagnoses of cancer to the central cancer registry within 180 days of diagnosis. The provider must report the patient’s name, address, gender, birth date, type of cancer, date of diagnosis and the name of the diagnosing physician. If the practitioner fails to report a reportable case to the registry in the appropriate format, the Oregon Health Authority’s Public Health Division will contact the practitioner and may access those portions of the relevant patient’s medical records if necessary to complete the information required for a reportable case.
