Occupational diseases – report by physician to department of health

Physicians must report to the department of health any patients whom the physician believes to be suffering from poisoning from lead, cadmium, phosphorus, arsenic, brass, wood alcohol, mercury, or their compounds, or from compressed air illness or other such occupational diseases and ailments within 48 hours from the time the patient was first treated.  The report must include:

• Name, address, and occupation of the patient;
• Name, address and business of employer;
• Nature of disease; and
• Such other information as may be reasonably required by the department. 

Reported information that is protected health information may only be released in accordance with the law governing protected health information; information that does not identify an individual may be released in summary, statistical or aggregate form.
