The director of a facility that provides services to the mentally disabled must notify a qualified person of an accident or injury that affects the health or safety of a patient receiving care and treatment at the facility, by phone, within 24 hours of the initial report of the incident. Upon request, the director will provide a written incident report, but will not include personally identifying information about patients or employees without their consent.
