This regulation requires that every Department of Health employee who has received training according to § 50-4.7 sign a statement acknowledging receipt of the rules that govern confidentiality and the fact that violation of confidentiality statutes and rules may lead to disciplinary action or criminal prosecution. The regulation requires that each signed attestation be maintained centrally in the employee's personal history file.
