The following external review requirements applies to all health carriers except long-term care insurers, Medicare, Medicaid, the Children’s Health Insurance Program, health care services provided by the Department of Corrections, the federal employees health benefits program, coverage offered to members of the armed forces, liability insurance, workers’ compensation, automobile insurance, or health care services not regulated by the state such as self-funded plans.
