“Authorizations under the Insurance Department regulations”
A valid authorization to disclose used by an insurer must be in written or electronic form and contain the identity of the enrollee, a description of the types of health information to be disclosed, a description of the recipients of the information, the enrollee’s signature, and notice of the length of time for which the authorization is valid and that the enrollee has a right to revoke the authorization at any time and the procedure for making a revocation.
