“Patient management under the division of public health services regulations”

Hospital admission procedures must include:

• Provision of information in a clear and concise manner to enable the patient to make appropriate treatment decisions;
• Collection a specific patient medical and social history information including information concerning advance directives or alternative decision makers; and
• Development of a treatment plan.
• Include provisions of information concerning patient rights and a language the patient can understand, which may require a translator.

Discharge documentation must include:

• A date and time of discharge;
• The status of the patient at the time of discharge; and
• Any discharge planning or referrals that have been conducted for the patient.
