“Required documentation under the division of elderly and adult services regulations”
Each participating provider of the Choices for Medicaid waiver program must maintain documentation for applicable licensure requirements and to support each claim billed for services. Documents must be retained for at least six years from the date of service or until the resolution of any legal actions, whichever is longer.
Each participating provider must also develop a written care plan that is developed in consultation with a patient or the patient’s legal representative. The provider must communicate with the patient’s case manager to ensure that the care plan is consistent with the service needs identified in the patient’s comprehensive care plan.
