“Elements of de-identification under the Commissioner, Department of Health and Human Services regulations”
A record presented to the New Hampshire healthcare quality assurance commission is considered de-identified of identifying an individual or facility if the following information is removed from the record: names; addresses; all dates relating to an individual including date of birth, date of death, etc; telephone and fax numbers; email addresses; social security numbers; medical record numbers; account numbers; health plan beneficiary numbers; certificate or license numbers; vehicle identifiers such as license plate numbers; device identifiers and serial numbers; web universal resource locators; internet protocol address numbers; biometric identifiers; full face photographic images; and any other unique identifying element.
