These requirements only apply to health care providers (and facilities) that are not considered covered entities under HIPAA.
A health care provider may charge up to 50 cents per page for a paper copy or photocopy of requested health care information. A provider may also charge up to $15 for searching for and handling recorded health care information requested by the patient.
