A health care provider may not disclose health care information without a patient’s written authorization.
A health care provider must retain, in addition to patient records, a record of each person that received or examined the patient’s health care information, for 3 years. The disclosure record must include the following:
· Contact information for person accessing the record
· Date of access
· Description of the information disclosed
