A physician or physician’s assistant must maintain a health care record on every patient. The records must be kept for at least 5 years from the last entry.
The patient health care record must contain relevant information to the patient’s medical condition, including medical history, diagnosis, and treatment plan. Each entry in the record must be dated and able to be understood by other practitioners.
