Mich. Comp. Laws Ann. § 333.16177 - License or registration; form of application; inclusion of social security number; examination; passing scores; additional information; exception to social security requirement

Michigan requires providers applying for or renewing their license to submit an affidavit declaring that they have a “written policy for protecting, maintaining, and providing access to” their medical records and for dealing with their medical records if they cease to practice. The department of licensing and regulatory affairs may request a copy of this policy.
