All patient records must be retained for 3 years following the end of services. The medical records must contain a signed consent form, the date of each visit, the amount of methadone dispensed, the results of urinalysis tests, and the client’s treatment plans, among other minimum requirements. Any deaths or premature births due to methadone treatment must be reported to the Food and Drug Administration within 2 weeks of the occurrence.
