The medical and health report required by Health & Safety Code §102450 (pertaining to live birth reporting requirement) must be labeled as confidential and the parent must be informed of the purpose of the information collection, that it is confidential and that it is voluntary. If the parent objects to providing information, that information should not be put in the report. The mother, father or child must not be identified by name in the report.
