Freestanding ambulatory care facilities must maintain medical records for each patient. Each patient’s medical record must contain patient identifying information, the patient's medical history, documentation of care provide, evidence of consent, and discharge diagnosis. A facility must send a copy of the medical record with the patient on referral to another health care provider or on transfer to a hospital.
