Information to be provided by licensees who personally furnish drugs to patients – Ohio Rev. Code Ann. § 4729.79

Each licensed health professional authorized to prescribe drugs who personally furnishes a controlled substance or other dangerous drug must submit to the state board of pharmacy the following information:
- Patient identification;
- Prescriber identification;
- Date drug was furnished;
- Indication of whether drug is new or a refill;
- Name, strength, and national drug code of drug furnished;
- Quantity of drug furnished.
The information must be submitted electronically, except where the board has granted a waiver allowing the prescriber to submit the information in another format.
