Individualized Plan of Care
Requires a case manager to prepare an individualized plan of care for each recipient of DD waiver services. Identifies the “team members” that the case manager must meet with in order to create the plan (e.g., the participant, providers selected by the individual, etc.). Specifies the information that each plan of care must include (e.g., the participant’s desired outcomes, risk factors, informed consent, etc.). Requires the care plan to be reviewed semiannually, at the request of a team member, or when the participant’s circumstances change significantly.
