This law requires that every
- physician,
- superintendent of a hospital,
- director of a local health department/local office of department of child services/division of disability and rehabilitative services,
- superintendent of a state institution serving the disabled, and
- superintended of a school corporation
who diagnoses, treats or provides care for a person with disability report the condition to the state department of health within 60 days.
The law further requires that every licensed physician and optometrist file reports regarding persons who are blind or have visual impairment with the office of the secretary of family and social services.
