Hospital registration and reporting requirements – Ohio Admin. Code 3701-59-05

By March 1st of each year, every hospital must register with and report to the department of health the following information for the previous calendar year:

• Information needed to identify and classify the hospital;
• Information on the type and volume of services provided by the hospital, including:

• Number of inpatient surgical cases;
• Number of outpatient surgical cases;
• Number of pediatric and adult cardiac catheterizations performed;
• Number of adult open heart surgical procedures and the number of pediatric cardiovascular surgery procedures performed;
• Number of surgical operating rooms by category;
• Number of patients treated in an emergency room and released;
• Number of patients treated in the emergency room who were admitted to the hospital;
• Information on the location, type and volume of services provided by satellite units, including the total number of patients treated on an outpatient basis by service type.
• The number of admissions, including transfers from other units, the number of days of patient care, and the number of beds in use for each category of care;
• The number of inpatient discharges for patients referred to or transferred to other providers, the number of patients expired and the total number discharged under any circumstances; and
• The county (or state if other than Ohio) of resident of patients at the time of admission, reported in the aggregate.

Satellite facilities and facilities that are certified by the centers for medicare and medicaid services independently from the hospital in which it is located must register with and report to the department separately from the parent facility.
