Healthy Rhode Island chronic care management program - R.I. Gen. Laws Ann. § 23-17.22-3

The director of health must establish “criteria for the healthy Rhode Island chronic care management program…” The program must address (1) a “broad range of chronic conditions;” (2) methods for identifying eligible patients; (3) processes for coordinating care; (4) methods for increasing patient and provider communication; (5) the use of educational, wellness, and clinical management tools; (6) the use of process and outcome measures; (7) methodologies for aligning payments and creating financial incentives for improved care; and (8) the sharing of enrollee data in accordance with federal law and in a manner that does not include patient identifying information.
