The provider of a health care service (HCS) must maintain a medical record for each patient for five years from the date of discharge; such record must document the patient’s needs and assessments and services rendered. Each provider must maintain an adequate medical record-keeping system that allows for use of the records to verify the information and reports required by state and federal law or regulation. The provider of a HCS must take appropriate measures to ensure the confidentiality of patient medical records, and may only disclose such records as authorized by the patient or allowed by state and federal laws and regulations.
